Appointments

We are currently offering phone and essential in-person consultations. All appointments are 15min intervals. If you feel you need for more time with the doctor, please notify reception staff when booking your appointment. Please call 4165 1288 to book an appointment and ensure your contact details are up to date when speaking

Longer consultations may be required for: procedures (incisions, sunspot removals etc.), multiple or complex concerns, mental Health Issues or the completion of forms.

Home Visits

Home visits are available for patients who are unable to travel to the practice due to ill health and are provided at the discretion of the doctor. If you need a home visit please discuss with reception staff.

Gayndah Community Health Service

All Allied Health services require a referral from the doctor. Specialists visit the Eidsvold Hospital every fortnight. Gayndah Community Health arranges these appointments for the patients.

Results

Appointments need to be scheduled at the practice with doctor to discuss/obtain your results (pathology, X-rays/ Ultrasounds and Specialist report). Reception staff cannot discuss specific results with you.

Please take your X-ray/Ultrasound with you when you leave the practice. You may need these for future appointments with other doctors/specialist to help with your health care.

Pathology

Sullivan and Nicholades and Queensland Medical Laboratories pathology offer a specimen collection service at the practice. Blood tests will be performed by staff or one of our doctors, daily from 8:30am.

Appointments for pathology services need to be booked with reception staff. All pathology collection is a bulkbilled, unless otherwise indicated. Please advise staff what type of tests you need when booking. (Normal or fasting blood test).

After Hours Care

If you require medical care outside normal practice hours or the practice happens to be closed due to unforeseen circumstances, please call Eidsvold Hospital on 41657100. If the care is not urgent you will be directed to the practice.

If you require an appointment after hours please discuss with the receptionist.

CTG (Closing The Gap)

The Eidsvold Family Practice is now signing up Indigenous and Torres Strait Islander patients for the Indigenous Health Incentive and Pharmaceutical Benefits Scheme Co-payment Measure. This incentive programme is for your doctor to help you manage your chronic disease/s and to make sure you get access to cheaper medicines. You normally have to pay some or all of the cost of PBS medicines and this is called a co-payment.

Diagnostic Imaging Services

The Eidsvold Family Practice offers a range of diagnostic imaging services including pregnancy ultrasound, blood collection under ultrasound, removal of foreign bodies & other general point of care ultrasound.
